Killing other characters is the other winning condition, so what is the problem with it?
It depends on luck and every player's strategy if a win by reaching 20 VP is realistically possible.
(Take a look at how you play. If you are the first in Tokyo, you can stay for a round and on your next turn deal damage to everyone else, then those other players will be rolling for hearts and not you. Then get out of Tokyo early enough so that you are not desperate to roll hearts.)

I don't have a problem with killing.
In a 5-6 player game, you can't stay in Tokyo once someone hits you since you will be dead or near death if you try to stay in for a whole round. The only time you can stay in for a whole round is when you are down to 2-4 players. This is where the imbalance happens. The player in Tokyo can roll 3 times without worrying about any "bad" dice. But the damaged players outside of Tokyo have to worry about rolling a punch and being forced to enter Tokyo with low health, which is almost always a death sentence. Why have that be part of the game's rules? What does it add? I'd rather roll a punch on the third roll and then make a decision on if I should punch or not based on whether I think they will leave Tokyo or not.
