In my opinion monster sidekick is a bit too poweful to let somone keep his "keep" cards.
I believe, that there just was not enough space to write everything down.
If you have a look at the yard you will see, that it's written pretty small.
We normally play it like the other "rebirth" card, which makes it more balaced, because of the team up thingy. ^^
ANd in addition the card is cheaper than the other rebith card, where you definitely have to discard all "keep" cards.
Besides for the addon we put every evolution card of the revived monster back on the deck. We to this for the otehr "rebith" card as well.
I know it's not written down in the rules...
