Realizing that you are looking at making a variant rule, the actual rules aren't really particularly different, up till point 3. If Tokyo and Tokyo Bay are attacked (as they are attacked simultaneously), either or both monsters can leave. As there really isn't a difference between Tokyo and Tokyo Bay, with the actual rules, you don't really need to move from Tokyo Bay into Tokyo--it only matters where you are if the game drops to 4 players (at which point, if someone is in Tokyo Bay, they immediately leave, unless the game drops to 4 players 'cause the monster in Tokyo dies, and then you really wanna stay, you can go to Tokyo).

Are you suggesting a rule that says if you are in Tokyo Bay, only the monster in Tokyo can leave during an attack, and then the monster in Tokyo Bay is forced to go to Tokyo (after being prevented from leaving even if they want, if they take damage)?

That seems overly harsh, as it will be really easy to end up dead in that situation; if you take damage while in Tokyo Bay, and then instead of being able to flee, you are forced to go stay in Tokyo; a lot of the time you'll end up dead.
