You are correct, if you are in Tokyo and you roll 1 claw, all monsters outside of Tokyo take one Damage.The advantage of Nova Breath is that you are damaging everyone, just like being in Tokyo BUT only the monster in Tokyo damages you. Plus you can heal, which you can't do in Tokyo.
