Almost always with 4+. I've noticed no balance problems with more players. The fact that there are only three cards up for purchase each turn, and you can only buy one each turn keeps things from getting unbalanced for the early players. The more significant factor is the order of the appearance of cards, which is part of the game.
I've found it more suitable (for my tastes) at all player counts, including a couple 2-player games I've tested out. I just like getting more cards in there to make every game combo-delicious. This variant does that. And ever game feels a lot more interesting and varied than any of the games I ever played with the standard rules.
