If two players are on the board in a 5-6 player game, and the player count drops to four or less, do you immediately pull the monster out of the Bay? Or do you wait for that player to decide to vacate?

I always played the latter way.

Also, should the one in Tokyo vacate, we always slid the Bay monster into that spot, unless both vacated at the same time.
I just double checked the rules. If two people are in Tokyo and the player count drops to 4 then the player in the bay is removed from Tokyo. As you mention if there are two players in Tokyo and the player in Tokyo City decides to leave when attacked then the player in Tokyo Bay takes Tokyo City and the attacking player takes Tokyo Bay.
