If there are players in Tokyo City and Tokyo Bay at the same time and someone outside of Tokyo deals damage, do they choose which player to deal the damage to within Tokyo or do they deal damage to both players in Tokyo?

Both players in Tokyo are damaged.

If someone outside Tokyo deals damage to both players, which I doubt is the way the game is supposed to be played, can both players in Tokyo flee at the same time? Ignore this question if you choose which player in Tokyo to deal damage.

Both players can flee.

Furthermore, if there are players in Tokyo City and Tokyo Bay and one of the players within Tokyo deals damage, do they deal damage to everyone outside of Tokyo AND the other player in Tokyo with them?

Only players outside of Tokyo are damaged. The players inside should play temporarily as a team.

Lastly, if someone is in Tokyo City, but nobody is in Tokyo Bay yet and a person outside Tokyo rolls claws to deal damage, do they deal damage to the person in Tokyo City or do they just take over Tokyo Bay and deal no damage in the same way that someone has to take over Tokyo City in the beginning of the game without dealing damage to anyone?

They both deal damage to Tokyo City and go to Tokyo Bay unless the player in Tokyo City flees in which case they deal damage and go to Tokyo City.

The only problem I can see is if the player in Tokyo City and Tokyo Bay team up, they could deal a ton of damage in no time. This brings me to another question. If both Tokyo locations are occupied and the number of players dips below 5, is the player in Tokyo Bay automatically forced out?

They can deal a ton of damage especially when sitting next to each other at the table. You are correct if the players dip below 5 then Toky Bay is emptied. That is why it is better to hold on to Tokyo City.
