In a nutshell, the game is "king of the hill". The game balances on the proposition that the Tokyo player(s) will always be outnumbered and therefore vulnerable, but tempted to stick it out and take their beatings for the sake of a few bonus points every few turns (and a retaliatory strike). The game is balanced to the extent that players cooperate to dethrone the current "king" of Tokyo. When the Tokyo player is not outnumbered (because only two players remain) then the rewards of Tokyo far outweigh the costs. You can't heal in Tokyo, sure, but two points every other turn is a gravy train to victory.
As another has said, making the reward one point per round would be a good house-rule for two-player games. Or having each player control two (three?) monsters at once. But the best answer may be to find a better two-player game and play that instead.
