I think you misunderstood my discussion. We don't focus on attacking unless someone is low, and people dying isn't a problem. It just seems that people dying is the typical ending to our game, though points are often near 20. In fact, the scenario you described where a person takes some chip damage and starts their next turn in Tokyo with about 6 health is typical. What you don't expand upon is what happens next.

For us, the person in Tokyo will attack since he can hit everyone, likely doing some damage, but the next person to go can almost certainly force them out so it is rare to stay in more than one turn, even in the scenario you described. Additionally, when they do leave Tokyo, they are going to be very low on health, and usually it will take them a couple of turns before they are really even capable of going back in so it is very difficult for anyone to get more than a few points from Tokyo at a time.

I also find it a bit funny that you say I should focus more on a turtle strategy when it seems that doing this has created a game you needed to modify with variants to fix.
