It helps if you post the card text that you have questions about. I don't have my cards with me, but I'll make an effort at answering.

I believe you only get one VP or poison counter.

I believe Telepath allows you to reroll an opponent's die, but I might be thinking of a different card.

Once again, without the card text handy, I think you get to place the token immediately.

You don't enter Tokyo until you are resolving the dice, so after all rerolls.

If I messed up any of those card questions, I'm sure someone else will be along shortly to correct me.
