It is a four player game with one monster in Tokyo.
A. You are outside of Tokyo and you attack the monster inside Tokyo?
B. You are inside of Tokyo and you attack the monsters outside Tokyo?

Our group plays it literally that the immediate neighbors (ie to the left and right) take 1 damage in addition to any damage they should have taken (if any). We don't think too mch about it since King of Tokyo is a simple light filler game, to be played for the mood more than anything else.
